Oddly, we have seen this tab a few times, but now it seems to be gone. What explains it coming and going? Do I have to enable something to make it appear?
I verified that all standard services (Data, Index, Query) are running on all 3 servers in the cluster.

Yes.
I figured it out. The console is giving this message… “Warning: Cannot communicate with indexer process. Information on indexes may be stale. Will retry.”
So I suspect the whole thing needs a restart to fix the indexer process.
Just noticed something else. There may be a bug in the console browser interface. The following URL correctly shows the Query tab…

Correct.
I reproduce the same behavior commented.
Is like using http://couchdb01sb:8091/ui/ use the previous UI without Query Folder.
