Java Client Getting Stuck / Lost after node removed

We have / had a 3 node cluster (now 2). The first node in the cluster failed, so we removed it and rebalanced it out. The cluster continued working properly. We are using Couchbase 4.1 on the server and the client is Java 2.2.5.

We have had an application fail, not being able to tolerate this cluster change. We now get:

The above message occurs for ports 8092, 8093, over and over.

Before that, everything was working fine until this single item was found:
